We are credentialed with most healthcare insurance plans and we will bill your insurance company as a courtsey.
Credentialed by most Auto Insurance, Worker’ Compensation programs and TPA’s.
Please supply a valid ID and an updated insurance card at each visit.
You can check to see if our practice is included in your health insurance network by visiting their website on the back of your insurance card. We are credentialed with the following insurances:

Your co-pay or deductible is determined by your insurance plan and benefits.
Many insurance plans require that you make a co-payment at the time of service.
Patients are responsible for their co-pay and any other unpaid ammounts leftover.
During your visit, our staff will be happy to assist you with determining your coverage and your out-of-pocket responsibility.
Your statements and office visit notes are easy to read and can be accessed online by going to the Patient Portal.
The Patient Portal is where you can access and edit your personal health information and make partial and full payments.
We are happy to file the claim for you, but the balance is your responsibility if your plan does not pay after 30 days.
Any amount not covered by your insurance policy is due immediately after you receive your first invoice from us.

Self-pay
Don’t have health insurance? Have a high-deductible plan? Do you have gaps in your coverage? Our self-pay prices are a cost-effective solution to that gets you the immediate, quality healthcare assistance you need for non-life-threatening illness and injuries.
We have very competitive self-pay prices for our Office Visits and for our procedures that we perform. These procedures are at a discounted price for self-pay patients.
